Customer Service Counter Closure

Effective immediately, the Customer Service Counter at the Tumwater, Town Center 2 site, will be closed to decrease the risk of spreading the novel Coronavirus (COVID-19).

Application Information

Washington State is currently under a governor proclaimed state of emergency, resulting in an increase to licensure application volume for some key professions. To provide the best customer service, please call 360-236-4700 or email questions to hsqa.csc@doh.wa.gov .

A customer service representative will email you with detailed instructions if anything is missing from your application. If you want to check the status of your pending application, please check the Department of Health’s Provider Credential Search.

Online Applications

Please review the DOH website for directions on how to apply for a credential through online services. https://www.doh.wa.gov/LicensesPermitsandCertificates/ProfessionsNewReneworUpdate/OnlineLicensing

If you meet the criteria below then you will not be able to use the online services and need to submit  a paper application   https://www.doh.wa.gov/LicensesPermitsandCertificates/ProfessionsNewReneworUpdate

  • You have previously applied for a license in Washington state and your application was closed as incomplete. You can check for a closed application on the Department of Health’s Provider Credential Search.
  • You don’t have sufficient information in the public record to verify your identity.
  • You are applying for reactivation of a previous license.

You will need mail your completed paper application and fee to:
