Reminder: Health Professional Shortage Area Survey

Please complete the Washington State Department of Health's Health Professional Shortage Area Survey.

Please read the following from the Department of Health regarding the survey: 

The Washington State Department of Health is conducting a short survey of the primary care, dental, and mental health physicians in the state. The information is needed for updates to the Health Professional Shortage Area (HPSA) designations.

HPSA designations are a requirement for more than 30 federal and state programs, such as:

  • National Health Service Corps loan repayment and scholarship programs
  • Centers for Medicare and Medicaid Services (CMS) incentive payments for physician services, and
  • Enhanced reimbursement opportunities for qualified rural health clinics.

Facilities and clinicians in the counties below should have already received a mailed invitation with a link to the online survey. You can also access the survey online at this site. Please complete as soon as possible after your receive the invitation. Your participation in this survey is very important, we cannot recommend designations without a high response rate. We appreciate your time and attention.

Counties currently being surveyed:
Adams, Asotin, Clallam, Clark, Columbia, Cowlitz, Ferry, Grays Harbor, Jefferson, Kittitas, Klickitat, Lewis, Lincoln, Pacific, Snohomish, Thurston, Walla Walla, Whatcom, Whitman


The other counties will be surveyed starting in 2019.
